Sample Loan Request Form

Sample Selection Request
* There is a $250 refundable deposit for sample loans, which is fully refunded upon timely return of samples, in perfect condition, with labels and tags affixed.  All samples are loaned for review purposes and must be returned.
Shipping Method Request
** Shipping rate will be quoted, based on requested shipping method, quanitity of samples, and shipping address.  Upon completion of this form, you will receive a credit card payment link for shipping cost, return shipping cost, and refundable deposit payment.

SAMPLE LOAN TERMS & CONDITIONS

  • Complete your sample(s) request in the form above.  Please provide collection name, design name, and color name.  All samples are 12”x12” or 20"x30” size.

  • S&H charges are not included for rug orders or sample loans.  Pricing is quoted, FOB NY. 

  • Woven Concepts will email a payment link for S&H charges as well as a $250 refundable deposit charge.  The deposit charge is fully refunded upon timely return of samples in perfect condition, with original labels and tags affixed.

  • S&H cc payment link that is emailed will include return shipping charges.  Return shipping label is included with original packaging.

  • Please use Woven Concepts return ship label and packaging to return ship all borrowed samples.

  • Return ship all borrowed samples to: Woven Concepts, 10 Daniel Street, Farmingdale NY 11735.

  • Sample loan terms are 7-10 days.  Timely return is considered to be “Woven Concepts' receipt of all returned samples within 7-10 days.”  For limited and time sensitive samples, Woven Concepts may request sample loan return within 2-3 days.

  • Due to the lengthy process of producing rug samples, we have limited sample availability.  Limited samples are also wait-listed to be reviewed by multiple customers. 

  • We appreciate your understanding and adherence to our sample loan terms and procedures.
